Eligibility Requirements:

  1. Must live in Maine;
  2. Lack sufficient personal and/or financial resources for in-home services;
  3. Be ineligible for Maine Care Private Duty Nursing/ Personal Care Services Program, Maine Care Home and Community Based Waiver, Maine Care Adult Day Health, Maine Care Consumer Directed Attendant Services Program or the Consumer Directed Home Base Care Program;
  4. Be at least 18 years of age;
  5. Agrees to pay co-payment; and
  6. Require cueing 7 days a week for eating, toilet use, bathing, and dressing; or
  7. Need limited activities assistance plus a one person physical assist with at least two (2) activities of daily living (ADL) in combination with at least one of the following:
    1. limited assistance plus a one person physical assist with an additional ADL or;
    2. physical assistance with at least one (1) instrumental activity of daily living or,
    3. a nursing service or other professional service, at least 1 day a week, that is, or otherwise would be, performed by or under the supervision of a registered professional nurse.

Helpful Hints:

Generally low-tech AT devices can be purchased on a limited basis, if identified as functionally necessary as required in the plan of care.

The major function of this program is to provide the necessary direct care services to assist an individual to remain in their home.

If a person is eligible for Maine Care, then he/she may not be eligible for this program.

Funding may be possible for AT services, if in addition to home care services, a particular AT service is identified as being necessary in order to maintain independence and safety.
